Liquid spreads sideways through the quilting and down into the padding, so the mark you can see is the middle of something several times larger. We run a UV torch across the mattress in a darkened room first, because urine and other organic staining fluoresce and the true extent shows immediately, along with older accidents nobody mentioned. Treating what the light shows rather than what the eye shows is the difference between the smell going and the smell returning.

Heat is the only thing that dissolves the body oil built up on headrests and arms, and strong suction is what pulls the solution back out rather than leaving it in the foam. That second part decides how your furniture looks the next morning, and in Florida humidity it decides whether it smells.

Everything else in your home can be rinsed or extracted until it runs clear. A mattress cannot, so the whole job is about using the least liquid that will work and pulling it back out. The failure mode here is not a dirty mattress, it is a damp one.
